RBT vs. BCBA Billing — Who Renders, Who Supervises

Understand the billing differences between RBTs and BCBAs in ABA, including supervision requirements and the impact on claims.

The Rendering vs. Supervising Distinction

In ABA billing, two types of providers appear on claims: the rendering provider (who delivered the service) and the supervising provider (the BCBA responsible for the treatment plan and oversight). Getting this distinction right is critical for accurate claims.

When the RBT Renders

When an RBT (Registered Behavior Technician) provides direct therapy, the claim is typically billed under CPT 97153 (Adaptive Behavior Treatment by Protocol). The RBT is the rendering provider, but the BCBA must be listed as the supervising provider.

  • Use modifier HM on the claim line (less than bachelor's degree)
  • Some payers require the BCBA's NPI as the referring/supervising provider
  • The BCBA must provide the percentage of supervision required by the BACB and documented in the chart

When the BCBA Renders

When the BCBA provides direct treatment (protocol modification, assessment, caregiver training), the BCBA is the rendering provider:

  • 97151 — BCBA assessment
  • 97155 — BCBA direct treatment with protocol modification
  • 97156 — BCBA caregiver training
  • Use modifier HO (master's level) or HP (doctoral level)

Supervision Ratios

The BACB requires BCBAs to provide a minimum of 5% supervision of RBT hours. Many payers have their own requirements (often 10–20%). Document supervision dates, duration, and type in the clinical record.
